WHEN YOU CHOOSE AGAPE SENIOR CARE HEALTH SERVICES INC.

• All personnel at AGAPE are independent contractors and are screened/credentialed in
   accordance with our high standards.
• Free hospital pickup upon discharge.
• A Registered Nurse will come to your home to perform an initial assessment.
• A Registered Nurse will consult with your physician and develop a plan of care specific to

   your needs.
•  Caregivers are carefully chosen to suit the needs of the patient.

• All caregivers, licensed and non-licensed, are regularly supervised and evaluated by 
  clinical management to assure that Agape's high standards of care are met.

• Coordinators are available 24 hours a day 7 days a week, including holidays.

• Agape Senior Health Care Services has over 25 years of experience.
